This questionnaire is an online assessment tool jointly developed by The Chinese University of Hong Kong, City University of Hong Kong, and Caritas Hong Kong Youth and Community Service Online Outreach Project. Its purpose is to provide more effective, timely, and clear crisis assessments, thereby assisting social workers in understanding the circumstances of service users and planning the next steps.
Participants should note that this questionnaire is solely a preliminary assessment tool and not a clinical diagnostic tool. Personal data collected through this project will be used exclusively for anonymous research to improve service quality. The project and the two universities are the sole administrators of the data. Upon completion of the research, all data will be properly destroyed.
